<Job Summary>

<Responsibilities>
- Conduct comprehensive patient assessments to identify social, emotional, and environmental needs, utilizing clinical counseling and diagnostic evaluation techniques.
- Develop personalized care plans that incorporate behavioral health strategies such as cognitive behavioral therapy (CBT), motivational interviewing, and group therapy.
- Provide individual and group psychotherapy sessions focused on mental health counseling, addiction counseling, PTSD care, dementia care, and working with individuals with developmental disabilities or autism.
- Facilitate crisis intervention and crisis management to support patients experiencing acute emotional distress or behavioral challenges.
- Collaborate with interdisciplinary teams to coordinate case management efforts, ensuring seamless integration of services across inpatient, outpatient, hospice, assisted living, and home & community care settings.
- Maintain accurate medical documentation and social work records in compliance with HIPAA regulations while utilizing electronic medical record (EMR) systems for data collection and record keeping.
- Assist with intake procedures for new patients and contribute to clinical research initiatives aimed at improving patient outcomes.
<Skills>
- Strong background in child & family counseling, behavioral therapy, and clinical counseling techniques.
- Experience working within hospital environments or inpatient settings; familiarity with nursing homes or assisted living facilities is a plus.
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
- Proficiency in working with individuals with disabilities, dementia (including Alzheimer’s care), PTSD, or autism spectrum disorders.
- Excellent communication skills for conducting diagnostic evaluations, motivational interviewing, and crisis intervention.
- Ability to manage sensitive information while adhering to HIPAA standards; experience with EMR systems is preferred.
