Medical Social Worker

<Job Summary>
Join our dedicated healthcare team as a Medical Social Worker, where you will play a vital role in providing compassionate psychosocial support to patients . Your expertise will help navigate complex emotional, social, and practical challenges associated with health conditions, ensuring holistic patient care. <Responsibilities>

  • Conduct comprehensive patient assessments to identify social, emotional, and environmental needs, utilizing clinical counseling and diagnostic evaluation techniques.
  • Develop personalized care plans that incorporate behavioral health strategies such as cognitive behavioral therapy (CBT), motivational interviewing, and group therapy.
  • Provide individual and group psychotherapy sessions focused on mental health counseling, addiction counseling, PTSD care, dementia care, and working with individuals with developmental disabilities or autism.
  • Facilitate crisis intervention and crisis management to support patients experiencing acute emotional distress or behavioral challenges.
  • Collaborate with interdisciplinary teams to coordinate case management efforts, ensuring seamless integration of services across inpatient, outpatient, hospice, assisted living, and home & community care settings.
  • Maintain accurate medical documentation and social work records in compliance with HIPAA regulations while utilizing electronic medical record (EMR) systems for data collection and record keeping.
  • Assist with intake procedures for new patients and contribute to clinical research initiatives aimed at improving patient outcomes.

<Skills>

  • Strong background in child & family counseling, behavioral therapy, and clinical counseling techniques.
  • Experience working within hospital environments or inpatient settings; familiarity with nursing homes or assisted living facilities is a plus.
  • Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
  • Proficiency in working with individuals with disabilities, dementia (including Alzheimer’s care), PTSD, or autism spectrum disorders.
  • Excellent communication skills for conducting diagnostic evaluations, motivational interviewing, and crisis intervention.
  • Ability to manage sensitive information while adhering to HIPAA standards; experience with EMR systems is preferred.
