Memory Care Manager

Responsibilities/Job Description:
Opened in 2020, the Pillars of Prospect Park is conveniently located off the light rail transit station and close to the metro bus near the University of Minnesota campus in Minneapolis, MN. This senior living community features 133 Independent living, 106 assisted living and 44 memory care apartments. Two buildings, make up this vibrant community and also encompasses an on-site Intergenerational Child Care Center. The Memory Care Manager is accountable for implementing the Dimensions Program, encouraging participation and ownership from all staff. Manages and supervises the programmatic, physical, and educational operations of Dimensions Community in meeting the resident's/family's needs and expectations. Collaborate with the interdisciplinary team and provide education to staff/families to adequately equip them to interact effectively with residents experiencing dementia.

This schedule includes;
  • 80 hours every two weeks ; full-time
  • Monday-Friday day shift
  • On call every 4th week
  • One weekend shift every 12 weeks (Manager on Duty rotation)

Responsibilities
  • Provide leadership, supervision, and coaching to staff, volunteers, and students.
  • Partner with the Director of Health Services (DHS) on staffing, orientation, training, performance feedback, and corrective action.
  • Maintain accurate department records and ensure proper documentation of activities and attendance.
  • Lead the implementation, development, and ongoing quality of the Dimensions Program.
  • Serve as the site's Dimensions Champion, attending required trainings and mentoring staff in best-practice dementia care approaches.
  • Collaborate with nursing to integrate CPT, My Way, Advance Care Planning, and GDS findings into resident care plans.
  • Promotes psychosocial, non-pharmacological interventions (e.g. aromatherapy, music, massage).
  • Ensure the environment supports engagement, dignity, and meaningful resident experiences.
  • Facilitate the Care Partner Support Group and send monthly Dimensions newsletters to families.
  • Assess residents' interests and needs; design and deliver programs that support independence and cognitive, social, and emotional well-being.
  • Plan, coordinate, and oversee daily activities, outings, and special events; ensure adequate supplies and staff support.
  • Collaborate with the Activities Director on calendars, staffing, hiring, and training.
  • Support marketing efforts, maintain current materials, participate in tours, and monitor occupancy.
  • Review admission assessments and collaborate on resident placement decisions.
  • Partner with the Executive Director and DHS on budgeting and financial monitoring.
  • Drive the Ebenezer bus for outings, ensuring safety, compliance, and accurate transportation logs.
  • Identify and report safety hazards, follow infection control and emergency procedures, and comply with all motor-vehicle laws.

Required Qualifications
  • A.A./A.S.
  • less than 1 year experience
  • Drivers License (in good standing)
  • Must be insurable under the employer’s/communities Auto Insurance Coverage
  • Medical Exam Certification (acquired during onboarding)
  • Must be at least 18-years old
  • Must complete and pass in-house transportation curriculum course and pass driving evaluation within 30 Days
Preferred Qualifications
  • Bachelor of Arts Degree
  • 1 year experience
Qualifications:
Compensation: $25.98 - $34.97 Hourly
