Job Summary

Position is responsible for the establishment and ongoing management of relationships with Memorial Hermann Physician Network (MHMD) physicians, primary care or specialty care, and their office staff to support Network expectations and drive performance outcomes. This position works with MHMD physicians to achieve value-based targets in quality, medical cost management and population health. This will involve identifying, tracking, and communicating provider issues and determining root cause and resolution approaches in consultation with MHMD, service line and care delivery site leadership.

MINIMUM QUALIFICATIONS

Education: Bachelor’s degree in related field preferred

Licenses/Certifications: (None)

Experience / Knowledge / Skills:

  • Three (3) years of healthcare experience with an understanding of medical practice operations required. This may include:

    • Practice management

    • Quality management

    • Business operations

    • Clinical operations

    • Population health and value-based care

  • Strong verbal/written communication and presentation skills

  • Interpersonal skills necessary to interact effectively and build strong relationships with physicians, staff and all levels of management

  • Ability to engage in constructive conversations with physicians to address performance and areas of improvement

  • Electronic medical record (EMR) proficiency and quality data fluency

  • Analytical skills necessary to identify customer needs and construct action plans

  • Working knowledge of Microsoft Office with analytical capabilities in Excel

  • Ability to drive to MHMD meetings, physician offices and care delivery sites

PRINCIPAL ACCOUNTABILITIES

Physician Onboarding:

  • Partner with MHMD, service line, and campus administrators to respond to MHMD membership applications

  • Establish Network performance expectations with physicians and office staff

  • Facilitate physician onboarding and training

Performance Support:

  • Serve as primary point of contact with MHMD physicians and office staff regarding Network expectations including value-based performance initiatives

  • Support deployment of MHMD programs and processes at the practice level

  • Perform monthly, in-person performance scorecard and data reviews

  • Maintain the requisite skills to analyze data and processes to recognize patterns in utilization and performance indicating opportunities or best practices

  • Facilitate action plan development based on feedback from physician scorecard reviews and MHMD administrative review forums

  • Identify and coordinate appropriate MHMD support services to support achievement of performance outcomes

  • Maintain consistent follow up on action plan progress and barriers to success

Health System Collaboration:

  • Travel to meet with MHMD physicians and office staff in collaboration with service line and care delivery site teams

  • Effectively communicate to physicians and office staff regarding MHMD and Memorial Hermann strategies and practice expectations related to strategic goals

  • Consult and partner with MHMD, service lines, care delivery sites and the practice to identify structural and functional challenges to performance improvement

  • Introduce and facilitate implementation of service line or care delivery site resources to support achievement of performance outcomes

  • Collaborate with MHMD on challenges impacting practice operations including credentialing, payer contracting, reimbursement and management services.

  • Collaborate with Community Connect Account Managers to identify and address training needs for practices, ensuring alignment with operational goals and effective implementation.

Reporting:

  • Document practice interactions, action plans, subsequent activities and outcomes

  • Escalate incidents regarding quality of care or physician concerns

  • Provide consistent reports/presentations to MHMD, service line and/or care delivery site administrators on action plan progress and barriers to success

  • Ensure documentation produced and/or processed complies with state/federal regulations and/or accrediting body requirements
