About the Role
We are a growing Physical Medicine & Rehabilitation (PM&R) and Pain Medicine practice seeking a motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a Skilled Nursing Facility (SNF) setting.

Locations
This role involves rounding at skilled nursing facilities across the following areas:
- Lodi, California
- Sonoma, California
- Davis, California
- Greater East Bay
Facility assignments are geographically clustered to minimize travel.

Key Responsibilities
- Evaluate and manage patients within skilled nursing facilities
- Diagnose and treat conditions including:
- Functional decline
- Musculoskeletal pain
- Spasticity
- Neuropathy
- Post-hospital rehabilitation needs
- Perform bedside procedures
- Conduct ultrasound-guided injections
- Collaborate with interdisciplinary teams (PT/OT/ST)
- Maintain accurate and timely EMR documentation
- Work closely with supervising PM&R and Pain Medicine physicians
Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
- Minimum of 2 years of experience as a mid-level provider (required)
- Required: Prior experience in PM&R and/or Pain Medicine
- Active DEA Registration (required)
- Strong clinical and diagnostic skills in a SNF or similar setting
- Ability to thrive in a fast-paced, high-volume environment
- Strong interpersonal and communication skills
- Procedural experience or strong interest in procedures
- SNF experience is a plus

- 1099 independent contractor position

Pay: $100,000.00 - $149,696.91 per year
