We are looking for a dedicated and patient-focused medical Provider to become part of our healthcare team. In this role, you will deliver high-quality clinical care, assess and manage a range of health conditions, and support patients in maintaining their overall well-being. Key Responsibilities:
- Travel with a Medical Assistant to conduct in-home patient visits
- Perform comprehensive patient assessments, including medical history reviews, physical exams, and necessary diagnostics
- Create and manage individualized care plans based on patient needs and conditions
- Carry out clinical procedures such as blood draws, IV therapy, injections, suturing, and related treatments
- Work closely with other healthcare professionals to ensure coordinated and continuous care
- Educate patients on diagnoses, treatment plans, and preventive healthcare practices
- Assess patient urgency levels and provide appropriate care or escalation when needed
- Assist with care transitions and coordinate follow-up or post-treatment support
- Stay informed on current medical practices and integrate evidence-based approaches into care delivery
- Maintain accurate and timely documentation of patient interactions and treatments
- Document patient visits and treatments accurately using electronic medical record (EMR) systems, including Practice Fusion
Qualifications:
- Medical degree from an accredited program
- Board certification in a relevant specialty
- Prior experience in a clinical or outpatient setting is an advantage
- Strong clinical skills, including experience with common procedures (e.g., phlebotomy, IV infusions, injections, suturing)
- Solid diagnostic and decision-making abilities
- Understanding of care coordination and discharge planning processes
- Familiarity with electronic medical records (Practice Fusion experience is a plus)
