Key Responsibilities:

  • Medical Records Management
  • Maintain patient files (inpatient & outpatient) in proper order.
  • Ensure completeness of records with necessary documentation, consent forms, discharge summaries, investigation reports, etc.
  • Check for missing or incomplete information and coordinate with nursing/clinical staff for rectification.
  • Coding & Indexing
  • Assist in coding diseases, procedures, and treatments using ICD coding systems (if applicable).
  • Classify and index records for easy retrieval.
  • Confidentiality & Security
  • Safeguard patient records as per hospital policy.
  • Ensure that records are accessed only by authorized personnel.
  • Data Entry & Digitization
  • Enter patient demographic and clinical details into the hospital information system.
  • Support in digitization of medical records and electronic health records (EHR) maintenance.
  • Legal & Compliance
  • Maintain records as per statutory requirements.
  • Provide records for medico-legal cases (MLC) with proper authorization.
  • Reporting & Coordination
  • Generate reports related to admissions, discharges, and patient statistics.
  • Coordinate with doctors, nurses, and administrative staff for record-related needs.

Skills & Competencies Required:

  • Good knowledge of hospital record-keeping systems.
  • Familiarity with medical terminology and ICD coding (preferred).
  • Strong organizational and documentation skills.
  • Attention to detail and confidentiality.
  • Computer proficiency (MS Office, Hospital Information Systems).

Job Types: Full-time, Permanent, Fresher

Pay: ₹10,000.00 - ₹12,000.00 per month
