Key Responsibilities:
- Medical Records Management
- Maintain patient files (inpatient & outpatient) in proper order.
- Ensure completeness of records with necessary documentation, consent forms, discharge summaries, investigation reports, etc.
- Check for missing or incomplete information and coordinate with nursing/clinical staff for rectification.
- Coding & Indexing
- Assist in coding diseases, procedures, and treatments using ICD coding systems (if applicable).
- Classify and index records for easy retrieval.
- Confidentiality & Security
- Safeguard patient records as per hospital policy.
- Ensure that records are accessed only by authorized personnel.
- Data Entry & Digitization
- Enter patient demographic and clinical details into the hospital information system.
- Support in digitization of medical records and electronic health records (EHR) maintenance.
- Legal & Compliance
- Maintain records as per statutory requirements.
- Provide records for medico-legal cases (MLC) with proper authorization.
- Reporting & Coordination
- Generate reports related to admissions, discharges, and patient statistics.
- Coordinate with doctors, nurses, and administrative staff for record-related needs.
Skills & Competencies Required:
- Good knowledge of hospital record-keeping systems.
- Familiarity with medical terminology and ICD coding (preferred).
- Strong organizational and documentation skills.
- Attention to detail and confidentiality.
- Computer proficiency (MS Office, Hospital Information Systems).
