Non-Medical/Medical Case Manager

Job Purpose

The goal of case management is to promote and support independence and self-sufficiency. Non-Medical Case Management (NMCM) and Medical Case Management (MCM) seek to enhance access and foster retention in medical care for people living with HIV through a range of client-centered services. These include community linkages to bio-psychosocial support for reducing real or perceived barriers to medical care.

Duties & Responsibilities

The Director of Case Management assigns duties to specific team members based on skills, experience, interests, and timing. Those assignments may change as our team develops and the needs of the organization change. The following is a list of the major projects and responsibilities of the NMCM/MCM. Other operations projects, both ongoing and one-time, will be assigned.

  • Requires regular travel in any of the 10 counties served for either transporting clients to medical appointments or completing paperwork for clients
  • Responsible for direct client service contacts and adherence to MDHHS Universal Standards and Standards for Medical and Non-Medical Case Management in Michigan
  • Functions as a member of a cohesive medical team to triage client needs, participate in program planning, designing, and implementing services. Reviews client cases each week and reviews care plans for efficacy
  • Successfully complete and maintain the HIV case management certification process through MDHHS
  • Maintain a timely documentation of interactions with people affected by HIV as well as other work-related activities
  • Refer individuals to support groups and other programs at Cares based on individual need or interest

Skills & Qualifications

  • Certified Community Health Worker (CCHW) with four years of providing direct HIV services and/or experience or associate’s degree with two years of providing direct HIV services and/or experience or bachelor’s degree in health or human services from accredited college or university
  • Experience enrolling individuals in health insurance plans, including eligibility assessment and application processing for Medicaid, Medicare and Marketplace
  • Can work independently but also function as a team member
  • Capable of working with individuals to identify needs and priorities and facilitate any positive change as a client defines that for themselves
  • Capable of taking the initiative and following a project through to completion
  • Capable of prioritizing needs, goals and objectives while also following a logical path to achieved priorities. Capable of maintaining all required information/data for ease of access by staff and clients
  • Utilize critical thinking skills to resolve problem situations as they arise. Able to identify root causes, assess relevant information, identify a range of options, and make recommendations for resolution of problem causes
  • Establish and maintain effective working relationships without judgment or bias to clients, staff, and other agencies that possess diverse social, cultural and economic background
  • Maintain the highest degree of confidentiality with regard to client and organizational information.
  • Must possess a valid drivers’ license and have access to licensed, insured transportation available during all regular working hours
