NURSE MANAGER – HOMECARE

We are seeking an experienced and highly motivated Nurse Manager with exemplary expertise in homecare services to lead and oversee our clinical homecare operations. The ideal candidate will ensure the delivery of high-quality, patient-centered care while maintaining compliance with regulatory and organizational standards.

Key Responsibilities:

• Provide leadership and clinical oversight to homecare nursing staff
• Ensure quality, safety, and continuity of care for home-based patients
• Develop, implement, and monitor care plans in collaboration with physicians and multidisciplinary teams
• Supervise staff performance, scheduling, training, and professional development
• Ensure compliance with healthcare regulations, policies, and accreditation standards
• Conduct patient assessments, audits, and quality improvement initiatives
• Serve as a key point of contact for patients, families, and healthcare partners
• Manage documentation, reporting, and operational workflows

Qualifications and Experience:

• Registered Nurse (RN) with a valid license (DOH/HAAD)
• Bachelor’s degree in Nursing (Master’s degree preferred)
• Minimum of [2] years of nursing experience, with significant experience in homecare or community health
• Proven experience in a leadership or nurse management role
• Strong knowledge of homecare regulations, standards, and best practices
• Excellent leadership, communication, and organizational skills
• Ability to mentor, motivate, and manage diverse clinical teams
• Proficiency in electronic medical records (EMR) systems

Preferred Skills:

• Experience in home health accreditation and compliance
• Strong problem-solving and decision-making abilities
• Compassionate, patient-focused approach to care
• Ability to work independently and manage multiple priorities
