Physician’s Services Group (PSG) is seeking a part-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our Post-Acute Care team in the Estill, SC area. This role offers schedule flexibility, clinical autonomy, and a collaborative care environment focused on improving outcomes for geriatric and post-acute patients within skilled nursing and long-term care settings.
Schedule and Work Environment

What You’ll Do
- Provide comprehensive primary and post-acute care to patients in skilled nursing and assisted living settings
- Conduct patient assessments, diagnose conditions, and develop treatment plans
- Manage chronic conditions and support preventative care initiatives
- Collaborate with physicians, nursing staff, and facility teams to ensure continuity of care
- Document patient encounters accurately and in compliance with clinical standards
- Help reduce hospital readmissions through proactive, patient-centered care
- Build strong relationships with patients, families, and facility staff

Qualifications
- Active Nurse Practitioner or Physician Assistant license in South Carolina, or ability to obtain
- Board Certified (AANP, ANCC, NCCPA, or equivalent)
- Strong clinical decision-making and documentation skills
- Experience in post-acute, skilled nursing, hospital, primary care, or outpatient settings preferred
- Interest in geriatric medicine, chronic disease management, and preventative care
- Ability to work independently while collaborating with a broader care team
