Nurse Practitioner (NP) / Physician Assistant (PA) – Post-Acute Care | Part-Time

Physician’s Services Group (PSG) is seeking a part-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our Post-Acute Care team in the Estill, SC area. This role offers schedule flexibility, clinical autonomy, and a collaborative care environment focused on improving outcomes for geriatric and post-acute patients within skilled nursing and long-term care settings.

Schedule and Work Environment

What You’ll Do

  • Provide comprehensive primary and post-acute care to patients in skilled nursing and assisted living settings
  • Conduct patient assessments, diagnose conditions, and develop treatment plans
  • Manage chronic conditions and support preventative care initiatives
  • Collaborate with physicians, nursing staff, and facility teams to ensure continuity of care
  • Document patient encounters accurately and in compliance with clinical standards
  • Help reduce hospital readmissions through proactive, patient-centered care
  • Build strong relationships with patients, families, and facility staff

Qualifications

  • Active Nurse Practitioner or Physician Assistant license in South Carolina, or ability to obtain
  • Board Certified (AANP, ANCC, NCCPA, or equivalent)
  • Strong clinical decision-making and documentation skills
  • Experience in post-acute, skilled nursing, hospital, primary care, or outpatient settings preferred
  • Interest in geriatric medicine, chronic disease management, and preventative care
  • Ability to work independently while collaborating with a broader care team
