Job Description

We are seeking an experienced Nurse Practitioner (NP) to perform initial patient assessments and follow-up visits across multiple skilled nursing and long-term care facilities throughout Connecticut. This role offers flexibility, strong clinical support, and minimal documentation burden with a dedicated medical scribe assigned to assist with charting.

The NP will travel to designated facilities over the course of the month to complete comprehensive assessments, establish care plans, and perform medically necessary follow-ups.

Key Responsibilities

• Perform initial comprehensive patient assessments

• Conduct follow-up visits as clinically indicated

• Review facility records, nursing notes, and patient histories

• Develop, document, and update individualized care plans

• Collaborate with facility staff and care teams

• Work alongside an assigned scribe for real-time documentation

• Ensure all documentation meets regulatory and billing requirements

Requirements

• Active Nurse Practitioner license in Connecticut

• PointClickCare (PCC) experience – required

• Experience in SNF, long-term care, geriatrics, or post-acute care preferred

• Strong clinical judgment and assessment skills

• Ability and willingness to travel to multiple facilities

• Excellent communication and organizational skills
