Nurse Practitioner / Physician Assistant

Job Summary
Allied Healthcare Physicians PLLC is seeking a compassionate and dedicated Adult Nurse Practitioner (ANP) to join our growing team. This role is ideal for an experienced clinician who is passionate about providing high-quality, patient-centered care in a collaborative and supportive environment.

As an Adult Nurse Practitioner, you will play a key role in managing the health and wellness of adult patients, focusing on preventive care, chronic disease management, and comprehensive treatment planning.

Key Responsibilities

1. Patient Assessment & Evaluation

  • Conduct thorough initial and follow-up assessments, including complete medical histories and physical examinations.
  • Identify symptoms, order and interpret diagnostic tests, and establish accurate differential diagnoses.

2. Care Planning & Management

  • Develop and implement individualized treatment plans tailored to each patient’s health goals and clinical needs.
  • Prescribe medications, manage ongoing therapies, and coordinate referrals to specialists when necessary.
  • Monitor patient progress and adjust care plans to ensure optimal outcomes.

3. Patient Education & Health Promotion

  • Educate patients and families about diagnoses, treatment options, and preventive care.
  • Provide counseling on nutrition, exercise, stress management, and medication adherence.

4. Clinical Procedures

  • Perform routine clinical tasks, including wound care, injections, and minor procedures.
  • Ensure proper documentation and follow-up for all interventions performed.

5. Care Coordination

  • Collaborate closely with physicians, nurses, and multidisciplinary team members to deliver seamless, integrated care.
  • Communicate effectively with patients and families regarding treatment goals and progress.

6. Documentation & Compliance

  • Maintain accurate, timely electronic health records (EHR).
  • Ensure all documentation complies with regulatory, legal, and organizational standards.

7. Quality & Professional Development

  • Participate in quality improvement initiatives and adhere to best-practice clinical guidelines.
  • Stay current with advances in adult primary care through ongoing education and training.

8. Administrative Support

  • Manage patient follow-ups, messages, and clinical inquiries.
  • Support efficient patient flow through scheduling and triage coordination.

9. Safety & Regulatory Compliance

  • Follow all state and federal healthcare regulations, infection control policies, and patient confidentiality standards.

Qualifications

  • Current and unrestricted Nurse Practitioner license in New York.
  • Minimum 1 year of clinical experience in adult or primary care preferred.
  • Strong interpersonal, communication, and clinical decision-making skills.
  • Proficiency in Electronic Health Records (EHR) systems.

Job Types: Full-time, Part-time

Pay: $130,000.00 - $146,000.00 per year
