Job Summary:
We are seeking an experienced and dedicated On-Call Dietitian . This role requires providing expert nutrition counseling and support to patients, clients, or residents on an as-needed basis. The On-Call Dietitian will be responsible for assessing clients' nutritional needs, developing personalized nutrition plans, and offering guidance on healthy eating practices to promote overall health and wellness.
Key Responsibilities:
- Patient Assessments: Conduct nutrition assessments for patients/clients based on medical history, dietary habits, and current health conditions.
- Nutrition Counseling: Provide personalized nutrition counseling and support to individuals and families, tailoring advice to meet the needs of various medical conditions (e.g., diabetes, hypertension, obesity, dialysis. etc).
- Develop Meal Plans: Create individualized meal plans, taking into account medical conditions, dietary preferences, and cultural considerations.
- Collaborate with Healthcare Providers: Work closely with doctors, nurses, and other healthcare professionals to ensure coordinated and comprehensive care.
- Education and Training: Educate clients, caregivers, and staff on proper nutrition, food safety, and diet-related disease prevention.
- Documentation: Maintain accurate and up-to-date records of nutrition assessments, counseling sessions, meal plans, and progress notes.
- On-Call Support: Provide immediate nutritional support for emergency situations, including patient care rounds, dietary changes, and other urgent needs as requested.
- Special Projects: Assist in developing nutrition-related educational materials, health promotion programs, and food service protocols as needed.
- Monitor and Adjust Plans: Track patient progress and adjust nutrition care plans as necessary, ensuring the best outcomes.
Qualifications:
- Education: Bachelor’s degree in Dietetics, Nutrition, or related field (Master's degree preferred).
- Certification: Registered Dietitian Nutritionist (RDN) credential required.
- Experience: Minimum of 1 years of experience in a clinical setting, community nutrition, or similar roles.
- Skills:
- Strong knowledge of medical nutrition therapy and evidence-based dietary practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficiency in nutrition software and electronic health record (EHR) systems.
- Availability: Must be able to respond promptly to on-call requests and work flexible hours, including evenings and weekends.
Preferred Qualifications:
- Experience in specific areas such as geriatrics, is a plus point
- Bilingual skills
Physical Requirements:
- Ability to lift and carry items
- Ability to stand or sit for extended periods while providing consultation.
