Job overview

We are seeking an experienced and dynamic Operational Lead for Patient Flow to drive the development, implementation and delivery of effective patient flow processes across Adult Community Services and Mental Health Services.

This is a high-impact role working across organisational boundaries, where you will support the creation of a seamless, responsive system that improves patient experience and optimises capacity across health and social care.

Main duties of the job

The post holder will be required to lead on the development, implementation and delivery of effective patient flow processes for Adult Community Services and Mental Health Services working across health and social care organisations.

The post holder will need to act as a conduit for patient flow between health and social care and be the first point of contact for inter organisational patient flow issues and to represent Cornwall Foundation Trust in communications with external agencies on matters relating to patient flow.

They will encourage integrated working between health, social care and the voluntary sector and lead on developing new and innovative ways of working in order to create an intelligent and robust system of capacity and flow.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Education / Qualifications and Relevant Experience

Essential criteria
  • Masters degree or relevant equivalent knowledge and experience
  • Experience of the delivery of managing patient flow and targets within a complex system
  • Evidence of formal post graduate training in management or health services management training
Desirable criteria
  • Significant Post Graduate Experience

  • Ability to communicate effectively using a variety of methods and to present contentious or complex information effectively at all levels of the organisation and with all levels of health and social care partners
