Peer Support Specialist / Case Manager

Liberty House is seeking a full-time Peer Support Specialist / Case Manager to join our dedicated team. In this blended role, you’ll combine peer mentorship with structured case management to support Veterans in achieving stability, independence, and long-term success.

At Liberty House, our mission is to remember, honor, and support our Veterans by connecting them to resources and providing a safe, substance-free transitional home. Join us as we walk alongside resident and community Veterans on their path toward self-sufficiency.

Responsibilities:

  • In this dual-impact role, you will:
  • Build supportive, trusting relationships with Veterans through mentorship and advocacy
  • Maintain an active caseload of residential clients, providing individualized coaching and goal planning
  • Collaborate with residents to create measurable action plans aligned with the Liberty House 4-Step Model
  • Connect Veterans to employment, housing, healthcare, and community resources
  • Assist with job placement and securing stable housing arrangements
  • Conduct and document regular drug and alcohol screenings
  • Accurately document case notes, progress, and service coordination in accordance with program policies
  • Engage with walk-in Veterans and connect them to appropriate supports
  • Develop and maintain relationships with local, state, and community partner agencies
  • Participate in program events, fundraising activities, and community engagement efforts
  • Maintain strict confidentiality and uphold the dignity of every Veteran served
Requirements:


  • We welcome candidates from peer support, case management, military, or human services backgrounds.
  • Associate’s or Bachelor’s Degree in Human Services, Social Work, Addiction Counseling, or related field required, Master’s preferred
  • 2-3+ years of experience providing direct services, coaching, case management, or similar support
  • Understanding of addiction, homelessness, and challenges impacting Veterans (PTSD, TBI, substance use, etc.)
  • Military service strongly valued but not required
  • Valid driver’s license required
