Per Diem Physical Therapist - Outpatient

Join our dynamic PT team as a Per Diem Physical Therapist specializing in outpatient care! In this role, you will deliver expert PT services to diverse patient populations, helping individuals regain mobility, reduce pain, and improve their quality of life.

This flexible position offers the opportunity to work across our 3 outpatient clinics on a Per Diem role! We have known coverage dates in 2026 for various staff vacations and maternity/paternity leaves, with potential for regularly scheduled shifts as the job evolves!

Duties

• Assess patients’ physical conditions through comprehensive evaluations, including mobility, strength, and functional ability.
• Develop and implement individualized care plans tailored to each patient’s needs, incorporating evidence-based physiotherapy techniques.
• Administer treatments such as trigger point therapy, manual therapy, and therapeutic exercises to promote healing and restore function.
• Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between inpatient, outpatient, home health, or community care settings.
• Educate patients and their families on injury prevention, rehabilitation exercises, and management of chronic conditions like orthopedics or geriatrics.
• Maintain accurate documentation of patient progress using electronic medical record (EMR) systems while adhering to medical terminology standards.
• Participate in ongoing professional development to stay current with advances in physiotherapy practices and healthcare regulations.

Experience

• Proven experience working in outpatient clinics preferred.
• Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice.
• Hands-on experience with discharge planning, care plans development, and managing diverse populations including geriatrics.
• Familiarity with WebPT EMR system a plus.
• Active NY PT license required

Pay: $40.00 - $50.00 per hour
