Job Title: Perinatal Navigator

Location: Must be physically located in the service area (Lee, Collier, Hendry or Glades County), must have the ability to work in the office, in clinic settings, and remotely, and attend in-person and online meetings as required

Starting Pay Range: $22 - $25 per hour, based on experience

This is a grant-funded position and is contingent upon funding award and continued grant availability.

Summary

Under the supervision of the Program Manager, this role provides direct case management and care coordination services to pregnant and postpartum women within healthcare clinic sites and community partner locations. The position is responsible for identifying, enrolling, and assessing clients; developing individualized plans of care; delivering education and referrals related to maternal and child health; and ensuring ongoing engagement through in-person and telehealth services. The role also serves as a liaison between clients, healthcare providers, home visiting programs, and community partners, while maintaining accurate data collection, documentation, and compliance with Healthy Start standards, contracts, and quality improvement requirements. Through outreach, education, and coordinated care, this position supports improved maternal and infant health outcomes and addresses social determinants of health. This role requires a proactive, detail-oriented individual who can work collaboratively with diverse teams and stakeholders to achieve our goals.

Essential Duties and Responsibilities include the following:

· Develop in-depth knowledge of Healthy Start Coalition of Southwest Florida’s programs, and local systems, partners, and services focused on maternal and child health, as well as the eligibility, referral, and follow-up processes for these programs and services.

· Participate in training and technical assistance related to telehealth program including, but not limited to, intakes, screenings, curriculum, referrals, databases, and outreach.

· Identify, recruit, and enroll referred telehealth program participants.

· Explain the purpose of the prenatal screening tool and encourage clients to complete the screening.

· Provide initial intake and assessment to new clients, addressing risks and determining the appropriate individualized plan of care for each client.

· Provide education, referrals, and services to participants regarding:

  • Home visiting programs, identifying a pediatrician or family practice provider, Medicaid and insurance navigator programs as necessary, Doula supports, breastfeeding support, WIC benefits as necessary, family planning, safe sleep, community resources, prenatal care, postpartum care, Social Determinants of Health, transportation, and other risk factors, importance of early literacy and infant brain development

· Provide information to prenatal women on signs and symptoms of preterm labor.

· Adhere to timeframes in compliance with the Healthy Start MomCare Network contract, the Department of Health contract, and the Healthy Start Standards and Guidelines.

· Enter and track client information and case notes into database systems.

· Perform in-person and telehealth wellness visits with clients to ensure ongoing engagement, enrollment, and identify further needs.

· Reassess risks and needs during each client encounter.

· Follow up on referrals made to ensure client connection to resources.

· Attend meetings and trainings, as required, including Coalition meetings and special events.

· Act as a liaison between medical professionals, clients, home visiting programs, and our collaborative partners.

· Stay current with literature relevant to maternal and child health, such as but not limited to disparities in health outcomes, infant mortality, maternal mortality, prematurity, low birth weight, maternal infections, maternal stress, smoking cessation during pregnancy, depression, optimal birth spacing, father inclusion, social determinants of health, substance use and exposure, trauma informed care, protective factors, safe sleep, and strategies for evidence-based interventions.

· Provide outreach to the community on issues related to maternal and child health, available programs and community resources through attending health fairs and other community events.

· Provide ICC services to women as outlined in the Healthy Start Standards and Guidelines and as required by current contracts.

· Create and mail provider letters.

· Maintain program supplies, inventory, distribution, and record keeping.

· Participate in continuous quality improvement activities.

· Maintain confidentiality of children, families, and other records and information in keeping with Coalition policies, practices, professional standards, and program requirements.

· Perform other duties as assigned.

Experience & Skills

· Bachelor’s Degree in social work, public health, health education, human services, health sciences, or a related field is required, and at least two years of relevant experience. Bi-lingual in Spanish and/or Creole is preferred. Certification as a Community Health Worker (CCHW) is preferred. Experience in community outreach, case management, and/or working with at-risk families is preferred.

· Knowledge of services, supports, and systems that serve children and families, particularly Florida’s system for maternal/child health.

· Excellent oral communication skills and ability to write in a clear, concise, and effective manner.

· Ability to work effectively with people of diverse backgrounds and demonstrate cultural sensitivity.

· Knowledge of and respect for confidentiality involving clients, stakeholders, and peers.

· Ability to establish and maintain effective working relationships within the community.

· Ability to work independently and collaboratively with limited supervision.

· Ability to plan, organize, and coordinate multiple work assignments and clients with an attention to detail.

· Ability to solve problems and meet deadlines in a fast-paced environment.

· Skilled in web conferencing, text apps, software and AI applications such as Microsoft Office for word processing, spreadsheets, and databases.

· Must have a valid driver’s license and reliable transportation, and be able to lift 15-20 lbs.

Employment Type

This is a full-time, non-exempt position. Standard hours of operation are M-F 8:30am-5:00pm. The schedule is flexible and may require some evening and weekend activities. Travel within Lee, Collier, Hendry, and Glades counties is required. Benefits include Paid Time Off, Holidays, Insurance and Retirement Plan.
