Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patients’ health status by checking vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals
• Support medication administration following prescribed protocols while maintaining HIPAA compliance to ensure patient confidentiality
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Utilize specialized equipment such as Hoyer lifts or transfer belts to safely move residents with limited mobility
• Maintain a clean and safe environment through regular cleaning routines and adherence to infection control standards
• Observe and document patient conditions, behaviors, and progress to update care plans and ensure personalized support

Qualifications

• Prior experience in home health, assisted living, nursing homes, or group home settings is highly preferred
• Knowledge of patient observation techniques and ability to monitor vital signs accurately
• Certifications in CPR and First Aid are required; additional training in Alzheimer's care, dementia care, autism support, or hospice care is advantageous
• Experience working with individuals with developmental disabilities or disabilities requiring specialized support is desirable
• Ability to perform heavy lifting and physically assist residents using equipment like Hoyer lifts safely
• Strong communication skills to collaborate effectively with healthcare teams, residents, and families
• Commitment to maintaining HIPAA standards and respecting client confidentiality at all times
