Personal Care Clinical Coordinator (LPN)

Location: Trillium Place
Current Opening:
Full‑time (evening shift, 2:00 PM to 10:30 PM)

Monday through Friday, no weekends.

The Clinical Coordinator is responsible for providing clinical oversight of residents in the Personal Care areas, ensuring consistent and accurate care is delivered, and that staff are properly trained and scheduled. This position will be performed in accordance with TANDEM LIVING Core Values, Mission Statement, and regulations.

QUALIFICATIONS:
Current PA license to practice as an LPN.
Current CPR certification.
Maintains current in‑person, American Heart Association–approved, hands‑on BLS certification.

Three (3) to five (5) years of experience in a health care environment, including leadership experience; long‑term care and assisted living facility experience preferred.

RESPONSIBILITIES INCLUDE:
Oversees the administration of medications and medication records to ensure accuracy, compliance, and timely delivery.
Secures prior authorizations for select medications for residents enrolled in PACE and SSI programs and serves as a liaison to pharmacy and TOCARE providers.
Reviews incident reports and resident charts, assessing for accuracy, follow‑up needs, and appropriate documentation.
Assists with assessments related to admissions, discharges, and level‑of‑care changes, ensuring clinical appropriateness and continuity of care.
Conducts clinical rounds twice per shift, identifying significant issues and communicating recommendations to the Senior LPN Clinical Coordinator and/or Director of Personal Care.
Provides training and serves as a clinical resource for Personal Care staff, fostering a safe work environment, supporting teamwork, and promptly communicating staffing or clinical concerns to leadership.
