Core Responsibilities

· Prescription Review & Dispensing:

  • Review and verify physician orders for accuracy, dosage, and potential drug interactions or allergies.
  • Monitor patient drug charts and manage complex treatment regimens.
  • Perform Medication Reconciliation by reviewing a patient’s full medication history to ensure safety during transitions of care.

· Patient Counseling: Educate patients on correct medication usage, possible side effects, and storage requirements.

· Medicine Management: Recommend optimal administration routes (e.g., tablets vs. injections) and assist in clinical decision-making with doctors and nurses.

· Inventory & Store Management: Manage the procurement and storage of pharmaceutical supplies, ensuring compliance with Schedule-P of the Drugs and Cosmetics Rules (1945) and maintaining "FEFO" (First Expiry, First Out) systems.

· Narcotics & Controlled Substances: Maintain strict records and secure custody (lock and key) for Schedule X drugs and poisons.

· Clinical Duties: Participate in ward rounds with doctors to recommend optimal administration routes and cost-effective alternatives.

o Monitor drug efficacy and report Adverse Drug Reactions (ADRs).

o Provide specialized support in intensive care (ICU), oncology, and emergency units.

· Quality Assurance: Ensure the identity, strength, and purity of medications through rigorous quality control procedures.

· Serves patients by preparing medications, giving pharmacological information to multidisciplinary health care team, and monitoring patient drug therapies.

· Prepares medications by reviewing and interpreting physician orders and detecting therapeutic incompatibilities.

· Dispenses medications by compounding, packaging, and labeling pharmaceuticals.

· Controls medications by monitoring drug therapies; advising interventions.

· Completes pharmacy operational requirements by organizing and directing technicians’ workflow, verifying their preparation and labeling of pharmaceuticals, and verifying order entries, charges, and inspections.

· Provides pharmacological information by answering questions and requests of health care professionals and counseling patients on drug therapies.

· Develops hospital staff’s pharmacological knowledge by participating in clinical programs and training pharmacy staff, students, interns, externs, residents, and health care professionals.

· Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by monitoring nursing unit inspection.

· Maintains records for controlled substances and removes outdated and damaged drugs from the pharmacy inventory.

· Supervises the work results of support personnel.

· Maintains current registration, studies existing and new legislation, anticipates legislation, and advises management on needed actions.

· Protects patients and technicians by adhering to infection-control protocols.

· Maintains safe and clean working environment by complying with procedures, rules, and regulations.

· Contributes to team effort by accomplishing related results as needed.

Educational & Legal Requirements

· Qualification: Minimum of a Diploma in Pharmacy (D.Pharm) or Bachelor of Pharmacy (B.Pharm) from a PCI-recognized institution.

· Licensure: Mandatory registration with the Karnataka State Pharmacy Council to legally practice in Karnataka.

Skill Set: Proficiency in pharmaceutical software, strong attention to detail, and excellent communication skills in both English and local languages.
