Patient Care Assistant is involved in both the clinical and administrative areas assisting behavioral clinic healthcare providers with patient care during face to face and telemedicine office visits; As well as handling clerical, environmental, and organizational tasks. The Patient Care Assistant works under general supervision of the nursing supervisor providing clinical support to clinic prescribers and nursing staff. Responsibilities include patient scheduling, and check in. Documenting individual care interventions, phlebotomy, and client education. Assisting with dynamic clinic flow, and other duties as assigned to meet complex needs for clinic and patients served.
Service Excellence

Initiate Medical Flow Sheet and Patient Health Questionairre-9 (PHQ9) at each encounter.
- Ensure Medical Flow sheet is completed accurately (perform and record Vital Signs) along with resource materials offered when indicated.

Promptly reports abnormal vital signs, lab results, and/or physical or mental health screening results that require further attention by the prescriber or nurse as indicated.

Assist in effectively setting up remote access connections between the prescriber and the individual as needed for tele-health visits.
Medication Related Services

Follow-up on medication calls and document inquiries appropriately in the electronic health record.

Monitor and manage refill requests.

Follows medication ordering refill protocols, verifying refill request in EHR and effectively troubleshooting refill request appropriately.

Long- Acting Injectable Antipsychotics Management
-
Prior Authorization for Medication Request submitted to individual’s insurer within 3 business days when injections have been prescribed when applicable.

Patient Assistance Program applications signed by individual and submitted to the Patient Assistance Program (PAP) specialist within 3 business days when applicable.
Phlebotomy Services:
- Perform venipuncture on individuals with active orders for blood and urine samples
- Identify, obtain and appropriately label all specimens collected (Blood and Urine).
- Maintaining respectful and professional boundaries during each encounter.
- All contaminated sharps and materials will be disposed of in accordance with LPS policies and procedures.
- Facilitate and coordinate laboratory testing services between LifePath Systems and contracted test performance laboratories (Quest).
- Responsible for ordering, stocking, and maintaining all supplies required for specimen collection.
- Responsible for tracking monthly absolute neutrophil count (ANC) results, maintaining the Clozaril Spreadsheet tracker, and faxing standing orders to external laboratory facilities as needed (LabCorp).

Maintains a clean working environment free of perceived safety concerns that may affect individuals receiving services and other workforce members.
- Provides support as needed to BH/Nursing (including but not limited to PAP, OBOT, OSAR, etc.).

Education, Training and Experience (including licensure and certification)
Patient Care Assistant
- Experience working with individuals with serious mental illness and/or substance use disorders strongly preferred.
-
2 years’ experience working in a clinic/hospital setting.
-
Knowledge of trauma-informed care and person-centered recovery.
-
Knowledge of the social services system and the ability to connect individuals with appropriate care.
-
Ability to inspire an environment that provides compassionate care to mentally ill individuals while maintaining confidentiality, respect, and dignity of everyone irrespective of their cultural background and severity of psychopathology.

Self-starter who works closely with all team members and takes initiatives in completing tasks.

Knowledge of and strong computer skills, including Microsoft Office Suite or similar software.

Excellent interpersonal, communication, and listening skills.

Ability to stand on feet, bend, and move throughout shift.
Phlebotomist
- Knowledge and understanding of standard specimen collection practices and procedures.

Strict adherence to safety regulations, operating and maintenance instructions, and procedure manuals.

Knowledge and understanding of aseptic and sterilization techniques.

High school diploma or equivalent.
-
Phlebotomy Technician certificate/license from The National Center for Competency Testing (NCCT), National Health Career Association (NHA), American Society of Clinical Pathology (ASCP), or the American Medical Technologists (AMT), preferred and required in some states.
