Key Responsibilities

• Evaluation: Perform comprehensive physical therapy evaluations to determine residents' functional status and rehabilitation needs.
• Treatment: Develop and implement individualized care plans to restore mobility, alleviate pain, and increase independence.
• Collaboration: Work as part of an integrated in-house team to ensure holistic resident care.
• Documentation: Maintain timely and accurate clinical records in compliance with facility and regulatory standards.
• Patient Advocacy: Act as a champion for resident health, safety, and functional improvement.

Qualifications

• Must possess a current, valid Physical Therapist (PT) license.
• Strong clinical reasoning and problem-solving skills.
• A positive, team-oriented attitude and a passion for helping others.
