Overview:
The Physical Therapist plans, organizes, and delivers comprehensive physical therapy services to support elderly and frail patients in maintaining independence, safety, and quality of life within the community. This role involves patient assessment, individualized treatment planning, hands-on therapy, and education of patients, caregivers, and families, while collaborating closely with a team to ensure coordinated, high-quality care across multiple settings.

Key responsibilities include:

  • Completing full patient assessments

  • Creating individualized plans

  • Providing treatment focused on chronic conditions common in geriatric populations

  • Chronic conditions are long-term health issues requiring ongoing management, such as diabetes, hypertension, pulmonary disease, kidney failure, multiple sclerosis, and Parkinson’s disease.

  • The overall goal is to keep patients as independent as possible, ensure they have access to all necessary resources, and reduce rehospitalizations by maintaining their health and safety in their homes.
    Candidates must also be comfortable responding to emergencies, acting quickly, and utilizing strong critical thinking skills.

  • Split time between the clinic and the field.

What you must have:

  • Advanced degree in Physical Therapy from an accredited school of Physical Therapy

  • Current Physical Therapist license in the state of practice

  • Valid driver’s license and proof of required auto insurance

  • Two (2) years of experience working within a team-based environment in a hospital, nursing home, or community-based setting preferred

  • One (1) year of experience working with a frail or elderly population preferred

  • If prior elderly or frail population experience is not present, training may be provided upon hire (if applicable)
