Essential Functions:
- Performs examinations and evaluations, and re-examinations , progress report of patients related to movement dysfunction with various diagnoses as applicable to the plan of care.
- Establishes a Physical Therapy diagnosis based on the examination and evaluation, organized in clusters, syndromes, or categories.
- Establishes a prognosis predicting the optimal level of improvement in function and amount of time needed to reach that level.
- Establishes a plan of care, and functional goals (specific, measurable, attainable, realistic, timely SMART goals) that incorporate the patient’s and/or family’s goals.
- Performs interventions which are purposeful and skilled, comprised of timely coordination of care, communication with parties involved in patient care, and patient/family education.
- Completes accurate patient documentation that justifies skilled need, according to Program and professional standards.
- Performs discharge planning and termination of formal intervention based on achievement of anticipated goals and expected outcomes, to facilitate a smooth transition to the next level of care.
- Meets Program productivity standards and outcome expectations (participates in data collection and analysis).
- Effectively directs and supervises support personnel (PT Assistant and PT Aide)
Skills:
- Be able to perform at the established guidelines for Program specific requirements.
- Bilingual (Spanish and English) is highly preferred or be able to communicate and professionally assess a a Spanish-speaking patient comfortably.
- Knowledge of WebPT electronic documentation is a plus.
- Competence in computers and computer applications (word-processing, google docs, excel).
Minimum Position Requirements:
Education: Require Coursework/Training - Graduate of an accredited (CAPTE) Physical Therapy professional program.

Licenses/Certifications:
- Valid, current California license.
- American Heart BLS Healthcare Provider level.
Preferred Position Qualifications:
Education: Master's or Doctorate Degree.
