Physical Therapist (PRN - Weekend Shifts)

The Lutheran Home is seeking a Licensed Physical Therapist to join our team on a PRN weekend basis. This role focuses on providing individualized rehabilitation services to support resident mobility, function, and independence. You will work in a collaborative healthcare environment guided by our values of service, excellence, dignity, fulfillment, grace, and stewardship. Responsibilities may include, but are not limited to:

• Plan, prepare, and carry out individually designed physical therapy programs to maintain, improve, or restore physical function, alleviate pain, or prevent physical dysfunction
• Perform and document initial exams, evaluating data to identify problems and determine diagnoses prior to intervention
• Evaluate treatment effects and adjust treatments to achieve maximum benefit
• Identify and document goals, anticipated progress, and re-evaluation plans in a timely manner
• Review referrals and medical records to help determine required physical therapy treatment
• Test and measure patient strength, motor function, sensory perception, functional capacity, or respiratory/circulatory efficiency and record data
• Perform all responsibilities following safety and infection control procedures
• Follow CMS, insurance, and AMA guidelines for skilled services, documentation, and billing
• Adhere to the Physical Therapy Statutes established by the Missouri Board of Healing Arts

• Licensed to work as a Physical Therapist in Missouri
• Experience in a skilled nursing facility preferred
