Position Summary

The Physical Therapist (PT) at Chestnut Park Rehabilitation and Nursing Center is responsible for providing high-quality, resident-centered physical therapy services to individuals requiring short-term rehabilitation and long-term care. The PT evaluates residents, develops individualized treatment plans, and implements evidence-based interventions to restore function, improve mobility, reduce pain, and maximize independence and quality of life.

The PT collaborates closely with the interdisciplinary team to support optimal clinical outcomes and ensure compliance with state and federal regulations.

Compensation

• Hourly Rate: $42.00 – $55.00 per hour
  (Based on experience, qualifications, and licensure)

Essential Duties and Responsibilities

• Perform comprehensive physical therapy evaluations upon admission and as needed.

• Develop and implement individualized treatment plans based on physician orders and clinical findings.

• Provide skilled interventions including:

  • Therapeutic exercise

  • Gait and transfer training

  • Balance and fall prevention training

  • Neuromuscular re-education

  • Manual therapy techniques

  • Functional mobility training

• Assess need for assistive devices and durable medical equipment (DME).

• Monitor resident progress and adjust treatment plans accordingly.

• Complete timely and accurate documentation including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with CMS, Medicare Part A and Part B guidelines.

• Participate in interdisciplinary care plan meetings and collaborate with nursing, social work, dietary, and activities staff.

• Provide education to residents and families regarding home exercise programs, safety, and discharge planning.

• Supervise Physical Therapist Assistants (PTAs) as applicable.

• Support facility QAPI initiatives and performance improvement efforts.

• Maintain infection control standards and adhere to facility safety policies.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current New York State Physical Therapist license in good standing.

• Current CPR certification.

• Experience in skilled nursing or geriatric rehabilitation preferred.

• Strong communication, organizational, and clinical decision-making skills.

• Ability to work collaboratively in a team-oriented environment.

Physical Requirements

• Ability to lift, transfer, and assist residents of varying mobility levels.

• Prolonged standing, walking, bending, and reaching.

• Ability to push and pull therapy equipment.

• Ability to respond quickly in emergency situations.
