Physical Therapist (PT) - Home Health *(Per Diem/Flex Schedule)*

Responsibilities of the Home Health PT (per diem - visits)

• Perform initial PT evaluation and develop care plans – non-OASIS
• Schedule patient appointments for therapy visits
• Perform PT visits according to the plan of care – non-OASIS
• Communicate with patient's MD as necessary
• Document care and communicate with the office
• Very limited need to visit our office

Requirements of the Home Health PT (per diem)

• Minimum 1 year of experience with direct patient contact in home health
• Current and unrestricted PT license in CA
• CPR and TB skin test
• Excellent interpersonal skills
• Valid driver's license, reliable car with auto insurance
• Proof of eligibility to work in the US
• Willing to submit to a background check
• Must pass drug screening according to federal guidelines
• Background check and drug screen
• Reliable transportation
