Mosaic Health is looking for an experienced Family Physician/Internal Medicine-Pediatrics focused provider to join our team. Mosaic Health, a network of nonprofit Federally Qualified Health Centers, provides primary medical, dental and behavioral health services in Western and Central New York State. This could be a full or part time opportunity in either region depending on the candidate’s desired location. Provider sign on bonus offered.
Candidates must have experience with team-based integrated care. In addition, an understanding of diverse patient populations from pediatrics to geriatrics preferred. Duties to include, but not limited to:
· Providing direct patient care
· Participating in call rotation with other Mosaic providers. Call on average is one week every two months and does not include in patient care
· Assuring patients are referred to other community providers as indicated
· Participating in Mosaic’s Quality Assurance program
· Providing support for Nurse Practitioners and Physician Assistants
· Works in conjunction with Medical Director and Chief Operating Officer regarding the development of new programs and revision of existing programs
Qualifications:
· Current valid license to practice medicine in the State of New York
· Board Certification in respective discipline
· Successful completion of all Mosaic credentialing requirements
· Familiarity with New York State Department of Health Article 28 rules and regulations and Public health background, cross-cultural experience and bilingual abilities preferred
· Experience with Electronic Health Record (EHR)
Our Central New York State location in Utica serves a diverse patient population. In addition, we operate a refugee health program that includes conducting Refugee Health Assessments for newly arriving refugees, as well as ongoing primary care, behavioral health and dental services for these and other members of the refugee community. Experience or comfort in working with translation service providers, refugee health program staff, and community-based organizations serving refugees is desirable.
In Rushville located Western New Work State, our patient population is composed predominantly of rural residents. The population includes a small share of patients who are most comfortable with Spanish as their primary language, thus requiring translation services or fluency in Spanish. We also serve the Amish and Mennonite communities.
