Physician – Skilled Nursing Facility

Job Description:

We are seeking a dedicated and compassionate Physician to provide high-quality medical care to residents in our Skilled Nursing Facility (SNF). The physician will be responsible for overseeing patient care, managing chronic and acute medical conditions, coordinating with interdisciplinary teams, and ensuring compliance with healthcare regulations. The ideal candidate will have a strong background in geriatric care, internal medicine, or family medicine, with a commitment to improving the health and well-being of elderly and medically complex patients.

Duties/Responsibilities:

• Provide comprehensive medical care to residents, including diagnosing and managing acute and chronic conditions.

• Conduct routine and as-needed patient assessments, adjusting treatment plans accordingly.

• Collaborate with nursing staff, therapists, social workers, and other healthcare professionals to develop individualized care plans.

• Oversee medication management, including prescribing, monitoring for side effects, and adjusting dosages as necessary.

• Ensure compliance with federal, state, and local healthcare regulations, including Medicare and Medicaid guidelines.

• Communicate with residents and their families regarding medical conditions, treatment options, and care goals.

• Provide palliative and end-of-life care management when appropriate.

• Participate in quality improvement initiatives, infection control measures, and staff education.

• Document patient encounters and treatment plans accurately in the electronic health record (EHR).

• Respond to medical emergencies and coordinate care transitions as needed.

Required Skills/Abilities:

• Valid and unrestricted state medical license to practice.

• DEA registration and ability to prescribe controlled substances.

• Experience in skilled nursing, long-term care, or geriatrics is preferred.

• Strong communication and interpersonal skills for working with patients, families, and care teams.

• Knowledge of Medicare, Medicaid, and SNF regulations is a plus.

• Ability to work independently while collaborating with an interdisciplinary team.

Education and Experience:

• MD or DO degree from an accredited medical school.

• Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related field (preferred).

Compensation: This is a 1099 position. Final rate will be discussed based on experience and availability.

Work Environment & Schedule:

• On-site at King David Center for Nursing and Rehabilitation (Brooklyn, NY).

• Flexible schedule options available (part-time or per-diem).

• Independent contractor (1099) status — no employment benefits.
