Primary Care Provider (MD/DO)

Janian Medical Care is seeking dynamic, collaborative, and mission-driven Primary Care Providers who are committed to delivering high-quality, person-centered care to individuals with lived experience of homelessness. The ideal candidate is passionate about community-based care, values interdisciplinary collaboration, and is motivated to help shape innovative services for some of New York City’s most complex and vulnerable populations. This is an excellent opportunity for clinicians interested in practicing in a supportive, learning-focused environment that prioritizes equity, professional development, and clinical excellence.

Duties and Responsibilities

  • Provide comprehensive, high-quality primary care to adults with lived experience of homelessness in community-based settings, including permanent supportive housing, transitional shelters, safe havens, drop-in centers, and through Street Medicine.

  • Deliver person-centered, trauma-informed, and evidence-based care to medically complex and vulnerable populations.

  • Collaborate closely with interdisciplinary teams, including psychiatry, behavioral health clinicians, social workers, case managers, and street outreach staff, to coordinate care and address medical, behavioral health, and social needs.

  • Participate in integrated care models through co-located clinical services and regular interdisciplinary collaboration.

  • Maintain accurate, timely clinical documentation, utilizing protected administrative time to support quality care and compliance.

  • Manage a balanced and sustainable patient caseload with a focus on quality outcomes rather than volume.

  • Participate in ongoing professional development activities, including weekly protected educational time and an accredited Grand Rounds series.

  • Engage in program development and continuous quality improvement initiatives to strengthen service delivery and client outcomes.

  • Uphold Janian Medical Care’s mission, values, and commitment to equity, social justice, and culturally responsive care.

  • MD, Board Certified or Board Eligible provider licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty
  • Good communication and collaboration skills
  • Commitment to person-centered practices
  • Commitment to servant leadership principles
  • Proficient in the use of electronic medical records (eClinicalWorks preferred)
  • Proficient in the use of Microsoft Office, including Outlook
  • Experience working with people living with mental illness and/or with homeless or justice-involved people preferred
  • Bilingual Spanish speaker preferred
