Population Health Coordinator

The Population Health Coordinator is critical to advancing value-based agreements (VBAs), the MSSP ACO program, and departmental operations. This role supports leadership with project coordination, strategy execution, and day-to-day operations. Acting as a subject matter expert on contract terms, quality expectations, and cost-saving goals, the coordinator ensures alignment with program objectives. Key responsibilities include quality chart reviews, data analysis, gap closure efforts, and leveraging population health tools to provide insights and optimize performance.

Responsibilities

  • Proactively provide quality chart review and direct submission to payor portals for gap closure.
  • Responsible for the development and management of monthly practice quality meeting reporting templates.
  • Monitor practice and provider progress with value-based agreements (VBA), analyzing reports, identify trends, and highlighting areas of opportunity.
  • Identify patients within the VBAs for inclusion in care management programs by analyzing clinical and utilization data.
  • Must be proficient with population health management tools to effectively provide the support and insights necessary to meet program objectives and practice needs.
  • Expected to serve as the subject matter expert (SME) on all contract terms, quality expectations and cost savings goals ensuring thorough understanding of requirements.
  • Assists in the development and implementation of population health strategies including community and patient outreach for gap closure.
  • Participate in the development of presentations and education materials needed to optimize VBR and MSSP ACO performance.


Qualifications:

Required

  • Bachelor’s or Associate’s degree in a relevant field (E.g., Business or Health Science)
  • Alternatively, acceptable certifications or license could be considered in place of degree.
  • At least one year of experience working independently with minimal supervision and meet project deadlines.
  • Demonstrated ability to manage multiple projects simultaneously, prioritize tasks effectively, and consistently meet deadlines.

  • At least one year of experience with Microsoft Office software (Word, Excel, PowerPoint, etc.)
  • Deliver relevant data and recommendations to Vandalia Health Network executive leadership.
