40 Hours/ Full Time/ Variable/ Day Shift/ Primary Care
- Our ambulatory clinics are located in Hyde Park and in West Roxbury.
- The Staff Nurse will be assigned by the Nurse Manager to work at both Faulkner Community Physicians (FCP) clinics in Hyde Park and in West Roxbury according to clinical and staffing needs, and it is essential and non-negotiable that nurse be available to work at both FCP locations as directed.
- Providing excellent nursing care with kindness and empathy to patients in the Community Health / Primary Care environment.
- Working collaboratively to provide excellent care to patients and families in the safest environment.
Job Summary
Summary The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department's Clinical Practice Manual. The registered nurse demonstrates initiative, knowledge, and clinical skills in caring for the primary care patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation, and evaluation of patient care and is directly responsible to a designated nurse manager or supervisor. CLINICAL PRACTICE
Organization of Patient Care
- Accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment.
- Sets priorities when organizing care for patients with varying acuity.
- Maintain continuity through clear and concise (verbal and written) communication.
- Demonstrates appropriate knowledge of growth and development of the adult and geriatric patient in the primary care setting.
- Quality of Care
- Documents patient care in a manner that is clear, complete, concise and in compliance with nursing documentation standards using SBAR communication.
Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records.
- Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department’s documentation standards.
- Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.
- Evaluates the effectiveness of the plan of care, and documents meeting stated goals in nurses' notes. Revises plan of care as needed to achieve desired outcomes.
- Participates actively in primary care clinics' quality improvement initiatives.
- Provides a safe environment for patients, staff, family and visitors.
- Administers medication safely according to established primary care policies and procedures.
- Performs nursing procedures safely and efficiently.
- Uses equipment safely and efficiently.
- Demonstrates awareness of potential/actual risks of infection and modes of transmission.

- Participates in the cooperative effort and peer support required for the smooth running of the clinic, flexibility in relation to patient care assignments, workday time assignment, and work schedule.
Education:
Graduate of an approved school of nursing or currently enrolled in a direct entry MSN program, with current registration in Massachusetts.
For newly licensed nurses a Bachelor of Science Degree in Nursing or BSN equivalency in a direct entry Master of Science Nursing program is required. Continued enrollment in MSN program is required up to completion.
Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
Experience:
1. Recent (within the past 1.5 years) experience in primary care setting is required.
2. Recent proficient telephone triage experience is required.
3. Recent nursing experience with caring and supporting the needs of gender-affirming patients is preferred.
4. Home Care experience preferred.
5. Ambulatory Certification preferred.
SKILLS AND ABILITIES:
Must meet all skills and abilities requirements on the BWH RN job description and additionally:
Demonstrated knowledge of both hospital-based and community-based care and resources required.
Clinical knowledge and expertise includes basic health teaching, family planning education, knowledge of advanced care directives, care for common illnesses.
Excellent communication skills, professionalism and organization required, demonstrates patience, compassion.
Excellent analytical, judgmental, and interpersonal skills and demonstrated ability to work effectively with patients, families and co-workers.
Certified Diabetic Educator preferred /Interest in becoming a CDE.
Motivational interviewing/Health Coach Certificate preferred /willing to achieve within two years of hire.
Technical dexterity, in addition to Epic, must possess a demonstrated ability to work with a variety of information systems and databases, including but not limited to: MS, Access, Excel - required.
Ability to be flexible and maintain equanimity in a fast paced, rapidly changing clinical setting – required.
The RN must show evidence of the basic analytic thinking necessary to care for a group of patients.
Must demonstrate observational skills and the ability to set priorities.
Must be able to function under stress using exceptional interpersonal and communication skills.
Must demonstrate effective skills in applying hospital standards in area of service, teamwork, communication, respect for others, and time/priority management.
