We serve seniors in private homes, assisted living, and memory care communities—bringing compassionate care directly to where it’s needed most.

Our physician-led model ensures patients are seen quickly, efficiently, and with continuity of care. We are expanding our primary and palliative care divisions and seeking a dedicated Nurse Practitioner or Physician Assistant to help us deliver comprehensive, home-based care to patients across the Phoenix Metropolitan area.

Job Description

We are seeking a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide in-home visits for patients with chronic conditions, serious illness, post-acute needs, and general primary care requirements. You’ll serve as the patient’s primary care provider managing their health holistically and building long-term relationships.

Responsibilities

· Conduct in-home visits across the Phoenix Metropolitan Area
· Provide primary care services
· Manage chronic conditions and preventive care
· Coordinate care with specialists, caregivers, hospice teams (when appropriate), and family members
· Document visits using eClinicalWorks (eCW) EHR
· Educate patients and caregivers on health maintenance, symptom management, and disease prevention
· Participate in care planning and interdisciplinary team meetings

Qualifications

· Certified Nurse Practitioner (FNP-C, AGNP-C, or equivalent) or Physician Assistant (PA-C)
· Active Arizona license
· Experience in home health, assisted living, palliative care, or primary care preferred
· Strong communication and clinical documentation skills
· Comfortable with mobile care and telehealth platforms
· Ability to work independently and manage a flexible schedule

Projected Total Compensation: $120,000.00 - $190,000.00 per year

License/Certification:

• NP or PA license in Arizona (Required)
