Job Summary
We are seeking a dynamic and compassionate Primary Mental Health Nurse Practitioner to join our multidisciplinary healthcare team. Duties
- Conduct thorough mental health assessments, including patient history, clinical examinations, and diagnostic evaluations.
- Develop and implement personalized treatment plans incorporating medication management, and lifestyle modifications aligned with best practices in behavioral health.
- Administer and monitor medications, including psychopharmacology treatments, ensuring adherence to HIPAA regulations and safety protocols.
- Collaborate with physicians, social workers, case managers, and other healthcare professionals to coordinate comprehensive care strategies across outpatient and telehealth settings.
- Educate patients and their families on mental health conditions, treatment options, and wellness strategies to foster engagement and recovery.
- Maintain accurate medical documentation, including progress notes, CPT coding for billing purposes, and ensure compliance with all regulatory standards.
Qualifications
- Current licensure as a Nurse Practitioner with certification in psychiatric-mental health (PMHNP) or equivalent credentials.
- Proven experience in behavioral health settings with familiarity in trauma-informed care approaches.
- Strong knowledge of mental health diagnoses using ICD-10 coding system; experience with ICD-9 is a plus.
- Proficiency in various EMR/EHR systems; telehealth experience is highly desirable.
- Demonstrated ability to perform comprehensive patient assessments including vital signs, mental status exams, and physiology knowledge relevant to psychiatric conditions.
- Must already currently have a supervising psychiatrist for collaborative practice.
- Experience working with diverse populations including children, geriatrics, individuals with developmental disabilities or dementia care needs.
- Ability to manage complex cases involving chronic illnesses like Geriatrics or memory care; proficiency in case management and discharge planning enhances service delivery.
