Who We Need
The ACT Team Psychiatric Nurse Practitioner is responsible for the provision of evidence-based and coordinated mental health services to participants in this program. The ideal candidate should have the ability to coordinate and work with a team of case management specialists in the screening of new referrals to the service, engagement of participants, comprehensive assessment, diagnosis, treatment planning, medication management, counseling, patient education, and risk assessment. The job requires coordination with the provider of medical care for the participants including their outpatient medical follow up as well as inpatient hospitalizations.
MINIMUM QUALIFICATIONS:
  • Education:
    • Completion of an accredited Psychiatric Nurse Practitioner program with PMHNP certification
  • Licensure & Certification:
    • Current Arizona state license
    • Board Certified (PMHNP-BC)
    • Valid DEA Certification
  • Experience:
    • Minimum of 3 years post-doctoral experience delivering psychiatric care to children, adolescents, and/or adults
JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
  • Serves as the Psychiatric Nurse Practitioner of all participants on the ACT Team and provides completes initial psychiatric assessments, progress notes, annual psychiatric updates, family interventions, develops differential diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, patient education, progress towards treatment goals including medical, psychological, social, behavioral and developmental aspects of the service participants’ situation.
  • Promotes and advances the principles and practices of recovery oriented treatment of ACT participants.
  • Documents participant's progress in the Electronic Health Record.
  • Maintains accurate, thorough, and current documentation in accordance with policies, procedures, and clinical standards of Copa Health. Consult complex cases with peers within the network and provide consultation and second opinions as needed.
  • Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for prior authorization for non-formulary medications as needed.
  • Discusses and follows appropriate process of obtaining consents for use of psychopharmacological interventions and other modalities of treatment.
  • Orders follow-up blood levels and supportive laboratory tests according to standard medical practice.
  • Participates in daily communication occurring at a level necessary to provide care for all service participants served at a level required for continuity of care across team principles.
  • Assesses service participants’ needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with outside providers according to established guidelines.
  • Available on-call 24 hours/day, 7 days/week unless otherwise scheduled as part of a rotation or in event of a scheduled absence in which coverage has been arranged.
  • Adhere to evidence-based practice and fidelity standards as required by AHCCCS.
  • Actively participates in clinical performance initiates to improve the quality of care of providers.
  • Provides necessary information to clinical team regarding inpatient utilization. Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for preauthorization of pharmacy services.
  • Participates in all Quality Improvement activities as required including but not limited to polypharmacy.
  • Works closely with other personnel to ensure that each treatment modality is appropriate for the service participants in addition to working with outside psychiatric providers in crisis and inpatient services.
  • Provides guidance in the process of obtaining, maintaining, or terminating court mandated treatments for participants.
  • Participates in medical staff meetings monthly medical staff meetings.
  • Works with Copa Health’s Medical Management and Quality Management to complete all deliverables (ACT Fidelity reports, COT reports, etc.). Participates in monthly and ad hoc meetings to address participants on the Copa Health High Risk and Emerging Risk Registries.
  • Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols.
  • Adherence to all agency policies, licensure, and training requirements.
