Quality Improvement Analyst

Job Responsibilities:

• Generate and manage outreach lists for patient engagement.
• Execute outreach campaigns for preventive care and chronic disease management.
• Maintain and update patient registries with Azara (or similar population health platform) for quality reporting.
• Monitor and reconcile Cologuard, laboratory, and diagnostic imaging orders.
• Ensure accuracy and timely processing of patient records and results.
• Process and track DME orders.Coordinate patient transportation services.

Requirements:

• Bachelor’s degree in healthcare administration, public health, or related field preferred.
• Prior experience in care management, quality improvement, or healthcare operations.
• Candidates must have good knowledge about eCW.
• Familiarity with population health tools (e.g., Azara DRVS) and electronic health records (EHR).

Skills:

• Strong organizational and data management skills.
• Ability to work independently and meet deadlines.
• Excellent communication and problem-solving abilities.

Details:

• Salary: 100k - 125k
• Timings: 6pm - 3am
• Office location: Phase 3, DHA Lahore.
