DRESS CODE: Business Casual (Jeans/T-shirts only on Friday with approval)
SCHEDULE: Full-Time, 40 hours per week; Monday–Friday 8:00 a.m.–5:00 p.m., with flexibility for evenings/weekends as needed based on program demands

Summary of Position

The Rapid Re-Housing (RRH) Case Manager provides short-term, housing-focused case management to individuals and families experiencing homelessness. The primary goal is to help participants quickly obtain permanent housing, stabilize in housing, and build the skills and connections necessary to maintain housing independently. This position utilizes a strengths-based, trauma-informed approach and reflects the short-term nature of RRH assistance. The RRH Case Manager works collaboratively with landlords, housing specialists (if applicable), and community partners to reduce barriers to housing placement and retention. The RRH Case Manager is responsible for housing search support, subsidy coordination, home visits, service planning, landlord mediation, and accurate documentation in HMIS and other required systems.

Essential Duties & Responsibilities

Services (approximately 70% of time):

  • Assist participants in locating and securing permanent housing as quickly as possible.
  • Conduct needs assessments focused on housing barriers (credit, evictions, income, criminal background, etc.).
  • Collaborate with housing specialists and/or landlords to facilitate housing placement.
  • Support move-in processes, including lease review, landlord communication, and housing inspections (if applicable).
  • Provide short-term, housing-focused case management aimed at stabilization.
  • Conduct regular home visits (minimum monthly or as program requires).
  • Develop individualized case plans focused on:
  • Lease compliance
  • Income growth and employment support
  • Budgeting and financial literacy
  • Housing retention strategies
  • Assist clients in accessing mainstream benefits and community resources (CalWORKs, CalFresh, SSI/SSDI, employment services, childcare, healthcare, legal services, etc.).
  • Mediate landlord/tenant conflicts when necessary.
  • Support participants in developing support networks beyond the program.
  • Increase intensity of services when needed to prevent housing loss.
  • Prepare clients for successful program exit and transition off rental assistance

Administrative (approximately 20% of time):

  • Maintain accurate and timely documentation in HMIS and agency databases.
  • Complete program eligibility documentation and re-assessments as required.
  • Submit rent assistance requests and required financial documentation.
  • Maintain organized client files in compliance with funder requirements.
  • Attend staff meetings, supervision meetings, and required trainings.
  • Track outcomes related to housing placement and retention.

Other Duties (approximately 10% of time):

  • Provide coverage support to other housing programs when necessary.
  • Assist with outreach efforts as needed.
  • Participate in community housing meetings or Coordinated Entry meetings as required.
  • Perform other duties as assigned by supervisor.

Qualifications

Education & Experience:

  • AA in Human Services, Social Studies, Social Work, Psychology, or related field (or equivalent combination of education and experience).
  • 1–2 years working with individuals experiencing homelessness and/or trauma.
  • Experience with housing programs, landlord engagement, or rental assistance programs preferred.

Skills & Abilities:

  • Understanding of housing-first and rapid re-housing principles.
  • Knowledge of landlord-tenant law and fair housing practices preferred.
  • Bilingual (English/Spanish) strongly preferred.
  • Proficiency in MS Office, Google Docs, and HMIS.
  • Strong communication, teamwork, and problem-solving skills.

Other Requirements:

  • Valid CA driver’s license, proof of insurance, and an insurable driving record.
  • Ability to travel locally for home visits and landlord meetings.
  • Must have a clear State DOJ and FBI criminal background check.
  • **Must be able to pass a pre-employment drug screen.**
  • Completion of 40-hour Domestic Violence Counselor/Advocate certification (State of CA).
  • Must complete SOAR training (within first year) and HUD Counselor certification as required.

Work Environment

The work environment includes a normal office setting, home visits, and community sites. Noise levels range from quiet to moderately noisy. This position requires flexibility in working conditions, including evenings and weekends as needed.

  • Manual dexterity required for repetitive motion using hands, wrists, and/or fingers for computer and office equipment.

Employment is At-Will. Wages are determined by experience and scope of position. Benefits such as vacation, paid sick leave, holiday pay, and health insurance will be discussed separately. Job descriptions do not constitute employment contracts.
