• Abdominal/ Vaginal hysterectomy*, Anterior/Posterior Repair (Colporrhaphy), Dilation & Curettage, Laparoscopic assisted gynecological procedures*, Abdominoplasty*, Breast augmentation*, Adult Dentistry*, Deciduous teeth extraction*, Amputations*, Ankle fusion*, Bunionectomy*, Hallux vagus arthroplasty/arthrodesis*, Appendectomy*, Cholecystectomy*, Hernia repair inguinal/umbilical*, Laparoscopic assisted procedures*, Mastectomy*, Bronchoscopy*, Colonoscopy*, EGD*, Circumcision*, Cystoscopy*, Nephrectomy*, Prostatectomy*, Code response*, Fire Safety*, Latex Allergy*, Malignant Hyperthermia*, Tracheostomy*.

Required Credentials:

• References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Unit/Facility Details:

• Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.

Special Requests:

• Pre-Approved Time Off: 1 Request.
• RTO must be on the submittal and cannot be changed once the profile has been sent to the manager for review. Please ensure all RTO is for the full 13 weeks for each submittal No RTO the first two weeks of an assignment.
