Registered Nurse Case Manager

Job Summary
We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our Home Health team. In this role, you will provide skilled nursing care to patients in their homes while coordinating and managing individualized care plans. The RN Case Manager works closely with physicians, therapists, and other healthcare professionals to ensure patients receive high-quality, patient-centered care that promotes independence and improves health outcomes.

Responsibilities

• Perform initial and ongoing patient assessments in the home environment.
• Develop, implement, and manage individualized care plans based on physician orders and patient needs.
• Coordinate patient care with physicians, therapists, home health aides, and other members of the healthcare team.
• Provide skilled nursing care, including wound care, medication management, and patient education.
• Monitor patient progress and adjust care plans as necessary.
• Maintain accurate and timely clinical documentation in the electronic medical record.
• Educate patients and family members on treatment plans, medications, and disease management.
• Ensure compliance with agency policies, Medicare guidelines, and regulatory requirements.
• Participate in case conferences and team meetings.

Qualifications:

• Active Registered Nurse (RN) license in the state of GA
• Graduate of an accredited nursing program
• Minimum of 1–2 years of nursing experience; home health experience preferred.
• Strong assessment, clinical, and critical-thinking skills.
• Excellent communication and organizational abilities.
• Ability to work independently and manage a patient caseload.

Valid driver’s license and reliable transportation required. Background and Drug Screen required.

Pay: From $50.00 per hour
