Position Summary

The Registered Nurse (RN) provides professional nursing services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings throughout Pennsylvania. The RN delivers skilled clinical care, implements individualized support plans (ISPs), conducts assessments, administers medications and treatments, and supports individuals and families in maintaining health, safety, dignity, and quality of life.

Position Scope

This position operates within the Nursing Department and reports to the RN Case Manager, Director of Nursing, or designated clinical leadership. The RN provides direct clinical services and collaborates with interdisciplinary teams to ensure compliance with Pennsylvania regulatory standards and OneWell Health Care policies. Services are delivered in participant homes and approved community settings.

Key Responsibilities

The Registered Nurse (RN) provides skilled nursing care to individuals with Intellectual and Developmental Disabilities (IDD) in home and community settings. Responsibilities include administering medications and treatments, performing clinical procedures (such as wound care and vital sign monitoring), conducting assessments, supporting chronic condition management, documenting care accurately, responding to medical concerns, and maintaining infection control standards. The RN collaborates with interdisciplinary teams, educates families and caregivers, participates in case reviews, and supports quality and compliance initiatives to ensure safe, coordinated, and person-centered care.

Confidentiality

The RN is responsible for safeguarding all Protected Health Information (PHI) and confidential participant information in compliance with HIPAA, Pennsylvania law, and OneWell Health Care policies. Unauthorized disclosure is strictly prohibited.

Documentation Requirements

Maintain accurate and timely clinical documentation in the designated electronic health record system.

Document assessments, interventions, medications, incidents, and care coordination activities.

Ensure documentation aligns with Individual Support Plans (ISPs) and nursing standards.

Complete required reports within state and organizational timelines.

Certifications

Current and active Registered Nurse (RN) license in Pennsylvania

CPR and First Aid certification (required)

Any additional certifications required by state or waiver programs

Education

Completion of an accredited nursing program (Associate or Bachelor of Science in Nursing) approved by the Pennsylvania State Board of Nursing.

Experience

Minimum of one (1) year of nursing experience required.

Experience supporting individuals with IDD, home health, or long-term care preferred.

Technical Skills

Proficiency in clinical assessment, medication administration, and nursing procedures.

Familiarity with electronic health record systems and mobile documentation platforms.

Ability to operate home-care medical equipment (feeding tubes, oxygen therapy, suction equipment, etc.).

Basic proficiency in Microsoft Office and internal mobile applications.

Languages

Ability to communicate effectively in English, both verbally and in writing. Additional languages are considered an asset based on participant population needs.

Driver License

Valid driver’s license in good standing and reliable transportation required. Must meet OneWell Health Care motor vehicle eligibility requirements.

Travel Requirement

Local travel within assigned Pennsylvania service regions is required. Travel includes participant homes, community settings, and occasional attendance at meetings or trainings.
