Job Title: Licensed Practical Nurse (LPN)

Location: Greenwood, IN

Employment Type: Full-Time
Shift: Monday–Friday, 8:00 A.M.–5:00 P.M. or Rotating Schedule

Job Summary:

Are you a compassionate and detail-oriented nurse passionate about behavioral health and recovery services? Join our team as a Licensed Practical Nurse (LPN) and make a meaningful difference in the lives of individuals receiving residential, outpatient psychiatry, addiction, and housing services.

In this role, you will provide direct nursing services within behavioral health and substance use treatment programs while maintaining a safe, therapeutic environment for clients. You will collaborate with medical and clinical teams to support medication management, health monitoring, and overall client wellness.

Key Responsibilities:

Medication Administration & Monitoring

  • Administer medications via oral, injection, and supervised self-administration per service line standards.

  • Conduct Medication Administration Record (MAR) reviews and medication counts.

  • Monitor medication stock and coordinate refills as needed.

  • Communicate medication concerns and refill needs to prescribing providers.

Client Assessment & Care

  • Perform nursing assessments, including physical and mental health screenings.

  • Room clients, verify medical history, confirm visit purpose, and obtain vitals (height, weight, BP, pulse, temperature).

  • Assist with lab procedures including blood draws and urine collection as needed.

  • Develop and document client safety plans and nursing-led interventions.

Documentation & Compliance

  • Maintain accurate and timely documentation of medication administration and nursing assessments.

  • Ensure medical and medication records are complete and up to date.

  • Adhere to all regulatory, service line, and organizational standards.

️ Health & Safety Oversight

  • Serve as a resource for nursing and medical concerns within the program.

  • Support infectious disease control, client hygiene, and overall health standards under nursing leadership guidance.

  • Participate in required meetings and collaborate with interdisciplinary teams.

Knowledge, Skills, and Abilities:

  • Strong documentation skills with proficiency in electronic health records (EHR).

  • Ability to assess clinical needs and provide constructive feedback to improve service delivery.

  • Excellent verbal and written communication skills.

  • Knowledge of confidentiality standards and HIPAA compliance.

  • Ability to interpret laboratory results, medical records, and procedural documentation.

  • Competency in basic mathematical calculations related to medication administration.

  • Strong problem-solving and adaptability skills.

  • Commitment to teamwork, professional growth, and culturally responsive care.

Education and Experience Qualifications:

  • Current, active Licensed Practical Nurse (LPN) license in the state of practice.

  • Graduate of an accredited practical nursing program.

Supervisory Requirements:

  • None

Employment Requirements:

  • Successful completion of background checks (criminal, driving, abuse/neglect, fingerprint).

  • Completion of New Hire Orientation and all required training (including Relias) upon hire and annually thereafter.

  • Current driver’s license, acceptable driving record, and auto insurance (if applicable to role).

Physical Requirements:

ADA Consideration – Sedentary Work

  • Exerting up to 10 pounds of force occasionally and/or negligible force frequently to move objects.

  • Repetitive hand and arm movements for typing and documentation.

  • Primarily seated work with occasional walking or standing.
