Job Summary
The Residential Care Coordinator oversees and coordinates all aspects of resident care services in the assisted living and memory care community. This role ensures compliance with state and HCQC regulations, manages care staff, and maintains a high standard of care that supports residents’ physical, emotional, and psychosocial well-being. Also, ensure the team and coordinate with other directors to maintain the good service as community provides.
Responsibilities
- Resident Care Oversight:
- Assess residents upon move-in and on an ongoing basis to determine care needs.
- Assess and have care conference update for residents when the condition change and/or routine ADLs update.
Team Management:
- Work under Care Director for lead the care team, coordinate and work together with other Directors to provide the best service to the residents
- Recruit, train and supervise care staff (med techs, caregivers).
- Conduct regular performance evaluations and coaching.
- Ensure proper staffing levels and manage daily care delivery assignments.
- Promote a culture of teamwork, accountability, and compassionate care.
- Monitor the care and service to match with care plan and caregivers and medtech follow their job task
- cover the floor if there is the call out from caregivers and medtech
Clinical & Medication Management:
- Supervise medication management program, including ordering, administration, documentation, and audits.
- Coordinate with physicians, home health, hospice, and other external providers.
- Respond to medical emergencies and oversee incident reporting and follow-up.
- Create the resident ADL plans
- Training and QC the care quality
Compliance and Quality Assurance:
- Maintain accurate documentation per state regulations (Nevada HCQC), including assessments, care notes, and incident reports.
- Prepare and participate in state surveys, audits, and quality assurance reviews.
- Ensure compliance with infection control and safety protocols.
- Ensure the on going training for the team to comply to regulations and suitable for quality services
Resident and Family Relations:
- Serve as the main point of contact for resident care concerns.
- Conduct care plan meetings with residents and families.
- Educate families on the progression of dementia or aging and community resources.
- First contact to handle and solve all the customer complain for resident care and comfortable living, bring to the management if it is unsolved or continuous complaints. And always inform the management and record incident report.
Collaboration & Communication:
- Partner with dining, activities, housekeeping, and maintenance teams to ensure holistic resident support.
- Attend interdisciplinary meetings and leadership team huddles.
- Maintain open communication with executive leadership regarding resident and team needs.
Qualifications:
- Education:
- Bachelor’s degree in related field preferred.
- Experience:
- Minimum 5 years (in total) of Caregivers, Medtech, Care coordinator in assisted Living and Memory care.
- Minimum 1-2 years of Care Coordinator experience in assisted living, memory care (preferred)
- Knowledge of Nevada Medicaid Waiver program and HCBS regulations a plus.
- Strong leadership, assessment, and problem-solving skills.
- Certifications:
- CPR/First Aid Certified.
- Valid Medication Technician

Working Conditions:
- On-call responsibilities including to fill in call out during weekday weekends and holidays.
- Fast-paced environment requiring flexibility and responsiveness.
- Regular walking, standing, lifting, and assisting residents.
- Develop and regularly update Individualized Service Plans (ISPs).
- Monitor residents’ conditions and make recommendations for care level changes or interventions.
- Ensure compliance with HCQC (Health Care Quality and Compliance), Nevada Medicaid Waiver requirements, and community policies.
Experience
- Previous experience in Assisted Living community, Memory care community, is prefeered and experience in home care, retirement homes, nursing homes, or group homes is a compulsory.
- Familiarity with dementia care practices and techniques is highly desirable.
- Background in Medicaid regulation is a plus.
- Ability to supervise and guide less experienced staff as needed.
- Strong interpersonal skills to build rapport with clients and their families.
- Knowledge of creating and following care plans is beneficial.
