About the Role

A leading acute care facility in Mission Viejo, CA is seeking an experienced Registered Nurse – Case Manager for a 13-week travel contract. This position offers the opportunity to work in a dynamic hospital environment with a strong interdisciplinary team. The RN Case Manager will be responsible for care coordination, discharge planning, and resource management to ensure patients receive safe and efficient transitions of care.

This role requires prior travel experience and at least two years of recent acute medical care management experience performing care coordination.

Key Responsibilities

• Coordinate comprehensive care plans for adult and geriatric patients across the continuum of care.
• Conduct patient assessments to determine post-acute needs and facilitate appropriate placements (SNF, DME, home health, etc.).
• Organize and participate in family and interdisciplinary care meetings.
• Collaborate with physicians, nurses, social workers, and other healthcare team members to ensure continuity of care.
• Arrange DME, ambulance transport, and SNF placements independently.
• Participate in developing individualized treatment plans and evaluating ongoing progress.
• Apply knowledge of HMOs, IPAs, worker’s compensation, and other healthcare payor systems.
• Ensure compliance with regulatory standards including CMS, HIPAA, OSHA, and The Joint Commission.
• Maintain accurate documentation using Epic electronic charting system.
• Float as needed to affiliated facilities (Laguna location).

Requirements

• License: Active California RN license (pending accepted if issued before start date)
• Experience: Minimum 2 years of recent acute medical care management experience performing care coordination
• Travel Experience: Required (Hard stop; no first-time travelers)
• Certifications: BLS (additional certifications preferred)
• Charting System: Epic (required)
• Weekend Requirement: Every other weekend rotation (varies to meet unit needs)
• RTO Restrictions: No more than 7 days approved per contract
• Orientation: 40 hours

Skills & Competencies

• Expertise in care coordination, discharge planning, and disease management
• Strong knowledge of CMS, HIPAA, OSHA, and regulatory compliance
• Familiarity with DRG and Core Measures/National Safety Goals
• Experience with patient populations including adults and geriatrics
• Ability to handle daily caseloads up to 25 patients
• Excellent communication, problem-solving, and time management skills
• Proficiency in Epic documentation and computerized record systems
• Capable of working independently in a fast-paced environment

Special Procedures & Details

• Case Managers do not perform Utilization Review.
• Must be proficient with assessing patients, organizing care coordination, and executing discharge planning.
• Expected to work with complex patient cases including those with homelessness, mental health crises, addiction, or requiring protective services.
• Knowledge of advance directives, POLST, foster/adoption processes, and drug/alcohol treatment resources preferred.

Compensation & Benefits

• Pay: $80.95/hour
• Overtime: $105.23/hr (1.3xhourly pay after 8 hrs/day or 40 hrs/week)
• Holiday Pay: $105.23/hr (1.3xhourly pay)
