Job Overview

"Navitas Healthcare, LLC" is an experienced Registered Nurse Case Manager to support care coordination in an acute care hospital setting. The ideal candidate will manage patient care plans, support discharge planning, and collaborate with multidisciplinary teams to ensure quality patient outcomes.

Key Responsibilities

• Coordinate and manage patient care plans throughout hospitalization
• Conduct comprehensive patient assessments and develop individualized care plans
• Collaborate with physicians, nurses, and interdisciplinary teams
• Facilitate discharge planning, referrals, and continuity of care
• Monitor patient progress and update care plans as needed
• Ensure compliance with hospital policies, insurance requirements, and regulatory standards
• Communicate care plans and resources to patients and families
• Maintain accurate and timely documentation of case management activities
• Support utilization review and resource management

Qualifications

• Active California RN license (required by start date)
• Minimum 2 years acute care case management experience
• Prior travel experience required (first-time travelers not accepted)
• Strong care coordination and discharge planning experience
• Ability to manage approximately 1:25 caseload ratio
• Excellent communication and organizational skills
• Ability to work in a fast-paced acute care environment
