RN/LPN Caregiver

Overview
We are seeking a dedicated and compassionate RN (Registered Nurse) or LPN (Licensed Practical Nurse) Caregiver to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess a strong clinical background, excellent assessment skills, and a genuine commitment to improving patient outcomes.

12 year old girl - Saturdays and Sundays 10am-6pm

LPN- $26-30 RN- $30-35

- Has Quadriplegic CP, Non-Ketotic Hyperglycinemia, and Seizures.

-Requires contiuous oxygen, g-tube feeds, medication administration, suctioning and assistance with ADLs.

Responsibilities

  • Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately following prescribed protocols while maintaining compliance with HIPPA regulations and medical documentation standards.
  • Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or those requiring hospice or palliative care.
  • Manage IV infusions, catheterizations, wound care, and tube feeding procedures with precision and aseptic technique.
  • Assist with diagnostic evaluations including specimen collection, laboratory processing, and imaging procedures like sonography or radiology as needed.
  • Support patient mobility and safety through proper use of assistive devices such as Hoyer lifts and ensure adherence to infection control practices.
  • Collaborate with multidisciplinary teams in settings such as hospitals (including ICU, trauma centers), nursing homes, assisted living facilities, or home care environments to develop individualized care plans.
  • Educate patients and families on health coaching topics including medication management, nutrition, chronic disease management, and discharge planning.
  • Maintain comprehensive medical records documenting patient care activities accurately in compliance with healthcare standards and ICD coding requirements for billing purposes.

Experience

  • Prior work in long-term care facilities including nursing homes and assisted living communities is preferred.
  • Experience with medical office procedures, triage protocols, and urgent or emergency room settings enhances candidate suitability.
  • Knowledge of ICD-9/ICD-10 coding standards for accurate medical recordkeeping and billing processes.
  • Demonstrated ability to work with individuals with disabilities or developmental challenges in home or community-based settings.
  • Background in pediatrics (including neonatal ventilator management), geriatrics, or post-acute care adds valuable versatility to your skill set.
