Duties & Responsibilities:

Position Summary: The Ryan White Case Manager has the objective of providing coordination, guidance, and assistance to improve access to and retention of needed medical and support services, and to mitigate and eliminate barriers to HIV care. The Case Manager provides comprehensive case management services to all Ryan White Eligible clients living with HIV seen at the VCU Infectious Diseases Clinic by performing assessments of the needs of referred clients and managing the care plan created.

Core Responsibilities:

Responsibility 1 (70%): Case Management

  • Screen for initial eligibility for the Ryan White grant program and annual renewals.
  • Initial assessment and identification of various service needs, referral to RW support services, and determination of referral to MCM.
  • Provide direct social work/case management services to clients living with HIV/AIDS and their families in accordance with their medical diagnosis, psychosocial, social, and financial needs. The scope of services includes screening, assessment, crisis intervention, adherence counseling, advocacy, information/referral, and care planning and coordination.
  • Formulate a plan of care with multidisciplinary teams to incorporate the physical, spiritual, and psychosocial aspects of the individual/family to promote optimal levels of wellness.
  • Complete and maintain documentation of all interactions with client/family to include psychosocial assessment and all ongoing interventions; all documentation will be completed in accordance with written clinical and administrative standards and the VDH Case Management Standards.
  • Respond to multiple referrals from community organizations, health department personnel, and community health providers to offer information regarding services.
  • Assures patient eligibility for grant-funded services is completed and up-to-date
  • Provide assistance with the referral process by following up on referral appointments.

Responsibility 2 (15%): Data Entry

  • Collect data and enter CAREWare data system in accordance with the time frame as stated in the CAREWare Data System Policies and Procedures, as set forth by the Data Manager.
  • Ensure accurate information into the EPIC, the electronic medical record.

Responsibility 3 (5%): Quality Management

  • Participate as required in Quality Management and Quality Improvement activities.

Responsibility 4 (7%): Other Duties as Assigned

  • Attend any supervisor-required meetings, trainings, and competencies
  • Attend one community event annually

Responsibility 5 (3%): Professional Development/Continuing Education

  • Attend or complete at least one pre-approved professional development/continuing education activity through the VCU Ryan White Programs Continuing Education (CE) Professional Development Policy

Qualifications:

Minimum Qualifications

  • 3 years of Case Management experience in a healthcare delivery system
  • Knowledge of Medicare, Medicaid, and other state or federally funded assistance programs for eligible individuals
  • Knowledge of HIPAA and ability to implement privacy and confidentiality practices
  • Excellent customer service and interpersonal skills
  • Excellent time management and organizational skills
  • Ability to demonstrate superior teamwork behaviors
  • Ability to work independently
  • Proficient in MS Office and data entry programs
  • Sensitive to confidential health information and HIV/AIDS related issues and controversies
  • Outstanding communication skills, oral and written
  • Cultural competency in working with a diverse population
  • Demonstrated ability to work in and foster an environment of respect, professionalism and civility with a population of faculty, staff, and students from all backgrounds and experiences, or a commitment to do so as a staff member at VCU.

Preferred Qualifications

  • Knowledge and Experience with the Ryan White grant program

Salary Range: $45,000-65,000 (Commensurate with experience)
