The Care Coordinator provides services for individuals with mental illness, substance use, or co-occurring disorders to assist them in navigating and access services through screening, coordination, and referral. Works as part of the Court Based Treatment and Diversion team to identify individuals in two local courts to coordinate services for clients, ensure compliance with court approved treatment plan. Also provides discharge services and active follow-up to engage the client in appropriate services. 
Starting Hourly Rate: $18.75 Hourly-Depending on Experience 
Education/Experience Required:
