The Senior Pharmacy Care Coordinator is responsible for providing technical and program support which includes but is not limited to: medication order review, pending lab orders, answering/routing incoming patient calls, in-basket management, supporting pharmacists and/or care teams, medication quality measures, prior authorization, and medication patient assistance programs. The Senior Pharmacy Care Coordinator's duties require interaction with clinicians, pharmacists, and other care team members.
Schedule: A 40-hour work week, Monday through Friday 7:00am to 6:00pm PST. The schedule will be determined by the supervisor upon hire.
Location: Remote with a commutable distance to the office at 1580 Valley River Dr, Eugene, OR 97401

Primary Responsibilities:
Patient Care
Patient outreach via telephone, MyChart, or other means
Provides basic technological support for patients (i.e. telehealth visits, signing up for MyChart, etc.)
Support scheduling/rescheduling of patient visits to ensure patients have follow-up
Provide patients with access to materials (i.e. mailing patient education handouts and required materials) and medications which may include medication assistance programs and foundations. Educate/mentor others on these programs and resources
Subject matter expert and resource to the pharmacist for insurance formularies. Contact insurance companies and pharmacies when necessary to determine medication coverage
Utilize medication knowledge to submit prior authorizations with appropriate medical documentation
Ability to resolve and mentor others on pharmacy/medication complex issues
Workflow Management
Manages incoming calls to the department, anticipating patient needs and proactively identifying solutions.
Manage incoming referrals and perform patient outreach
Management of telephone encounters and staff messages in pools/in-basket
Requests pharmacist intervention or clinician escalation when needed
Tracks and retrieves patient lab results
Track, inform, and obtain relevant information for care team of patient discharge from acute care facilities (i.e. hospital, skilled nursing)
Leads and implements quality measure processes which include programs such as 5-Stars (i.e adherence, blood pressure, A1c control) with the ability to work independently and solve complex challenges with suggested solutions
Performs other duties as assigned

The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
