Senior Scheduler

Suncoast Hospice, a part of Empath Health is seeking a Senior Scheduler. The Senior Scheduler has responsibility for coordination of patient care through management of patient visit schedules for multiple disciplines, including physicians, nurses, C.N.A.’s, social workers, chaplains, therapists, etc., within the Electronic Medical Record (Home Care Home Base), while ensuring compliance with care plans and payer requirements.

What you'll Do

  • Supports and displays the Empath Health mission, vision, and values.
  • Ensures adherence to CMS and payer guidelines for each discipline and develops visit schedules based on caregiver availability based on those guidelines to manage and maintain reimbursement compliance.
  • Updates and maintains accurate scheduling systems and the EMR (Home Care Home Base) in an accurate and timely manner.
  • Update team managers as needed to discuss scheduling conflicts and coordinate with all disciplines to integrate all into the patient’s plan of care.
  • Develop and implement best practices for reducing scheduling conflicts and to optimize staff utilization on an ongoing basis.
  • Generates reports from Scheduling Systems and Home Care Home Base (EMR) as needed to support continuity of care and identify gaps to be shared with team leadership.
  • Communicate with Care Team Manager, team Leadership and members of the interdisciplinary team to ensure visits are scheduled in a compliant and expedient manner.

What You'll Need

  • Education and/or Experience: High school diploma or general education degree (GED) required; Associates Degree in Health Care Administration preferred
  • Minimum of 5 years scheduling and/or administrative experience in hospice, palliative care, or related medical field required
  • Excellent documentation and organizational skills, including time management, multi tasking and prioritizing
  • Exhibit self-direction, independent judgment, and discretion
  • Displays attention to detail to ensure accurate documentation and compliance with care plans and payer requirements
  • Strong communication skills needed to communicate with patients, caregivers, and healthcare professionals
  • Proficiency with Microsoft Suite, Scheduling Software, and Home Care Home Base (EMR), required, with minimum of 5 years direct experience working within Home Care Home Base, strongly preferred.
  • Medical terminology and coding strongly preferred
