SENIOR SOCIAL WORK SPECIALIST.
The Senior Social Work Specialist leads and manages complex social work programs and initiatives, performs advanced assessments and interventions for patients with diverse needs, and spearheads innovative strategies and policies in social work. This role includes supervising and mentoring junior staff and representing the organization at high.-level meetings and conferences.
- Communicate relevant social factors to medical staff for integrated care

Participate in discharge planning and connects patients with community resources
- Engage in ongoing training quality improvement initiatives and disaster response efforts while preparing statistical reports on service delivery
- Maintain knowledge of external rehabilitative agencies and supervises junior social workers on complex cases
- Provide crisis intervention outside regular hours contributing to better patient outcomes and resource efficiency
- Respond to social work consultation referrals from physicians

Conduct counseling homesocial evaluations financial assessments and discharge planning forinpatients at JHAH facilities

Provide psychosocial assessments and counseling develops management plans and Assist healthcare providers in addressing psychosocial issues affecting medical outcomes

Coordinate with patients relatives and other significant individuals to facilitate adjustment to illness documents all interactions in Epic and follows up to ensure continuity of care
Master’s Degree in Social Work, Sociology or psychology from recognized univsersity
Social Worker Certification as approved or granted by the SCFHS
Minimum 15 years' experience (ideally in a healthcare setting)
