We are seeking a compassionate and dedicated Service Provider to join our team in delivering exceptional care and support to individuals in various care settings. Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patients’ vital signs and observe for any changes in health or behavior, documenting findings accurately
  • Administer medications and follow care plans under supervision, ensuring compliance with HIPAA regulations for patient privacy
  • Support clients with meal preparation, feeding, and nutritional needs to promote healthy living
  • Utilize specialized equipment such as Hoyer lifts or wheelchairs to safely transfer individuals with mobility challenges
  • Maintain a clean, safe environment by performing routine cleaning tasks and adhering to infection control protocols
  • Provide companionship and emotional support, especially for individuals with dementia, Alzheimer's disease, or developmental disabilities
  • Collaborate with healthcare professionals to develop and implement personalized care plans tailored to each individual’s needs

Experience

  • Prior experience working in assisted living facilities, nursing homes, or home health environments preferred
  • Hands-on experience with patient observation, medication administration, and vital signs monitoring required
  • Knowledge of dementia care, memory care techniques, Alzheimer’s care, and hospice care practices highly desirable
  • Familiarity with working with individuals with developmental disabilities or autism spectrum disorders is a plus
  • Certification in CPR and First Aid is required; additional certifications such as HIPAA training or nursing assistant credentials are advantageous
  • Experience handling heavy lifting safely using equipment like Hoyer lifts or similar devices preferred
  • Demonstrated ability to work compassionately with diverse populations while maintaining confidentiality and professionalism
  • Bilingual Spanish and English
